Details:
Location: Phoenix, Arizona 85009 (where the heat is high, but your impact is even higher)
Compensation: $60,000-$80,000 annually
Qualifications:
LMSW license-a symbol of your dedication
Master's degree in Social Work
Minimum 1 year of clinical social work experience-a testament to your commitment
Problem-solving skills with a positive attitude
Mission-driven attitude-a crucial trait for changing lives for seniors
PC proficiency
Fluency in Spanish or other languages spoken by people in the communities we serve

Your Daily Adventures:
Develop relationships and collaborate with the primary care teams to conduct social services assessments to determine the appropriate needs for each member
Plan, coordinate, manage and implement support packages to help members deal with social economic and medical barriers
Navigate managed care plans for community services and programs
Empower members to take charge of their health journeys
Document your impact with meticulous detail in clinical management systems
Guide members and families through advance care transitions with compassion
Unlock state-based prescription programs and other benefits to enrich our members' lives
