Adecco US, Inc. Practice Transformation Manager (hybrid role) in Long Island, New York

A Practice Transformation Manager (hybrid role) in Long Island, NY is now available through Adecco Medical and Science. In this role, you will report to the Title/Business Entity with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet requirements. The hours are Monday-Friday from 8:30am-5:00pm. The hourly pay rage is $ 57.69/hr.

Job Responsibilities

· Assist and support the practice in maximizing available information technology resources in order to achieve improved patient outcomes and increased office efficiencies in areas.

· Manage a team of Practice Performance Facilitators/Practice Transformation Providers across various practice settings.

· Population Health Management.

· Registry and enrollment functionality, including DCE voluntary alignment activities.

· Planned and walk-in care throughout.

· Care management and care coordination strategies.

· Patient action planning and goal setting in order to improve cost, quality and patient experience.

· Outcomes reporting.

· Patient safety

· Design, development, and implementation of community learning forums.

· Serves as the subject matter expert and liaison with Management to facilitate clinical processes between the organization and the practice, creates and hosts tailored learning opportunities to support the deployment of program interventions, use of support tools and other data resources, and provides technical support for such tools, as required.

· Drives success in population health performance by guiding care teams (practice physicians, staff) towards best practices, identifying gaps both through data and operational observation, by providing feedback, education, and training.

· Obtains and analyzes practice specific cost and quality data and reports for cost of care and quality opportunities and assists practice to target high risk members and gaps in care based on data/reports provided.

· Build trust-based relationships with practices and care teams to facilitate change management, engagement, solutioning, and sustainability of population health initiatives.

· Deliver progress reports and review with practices and care teams.

· Identify performance gaps from report data and guide care teams towards solutions.

· Identify and prioritize key quality and utilization initiatives critical to success under performance-based programs.

· Co-facilitate and/or lead practice transformation forums and meetings related to value-based care initiatives.

Qualifications :

Education/Experience :

BA/BS, Masters in public healthcare, healthcare economic, or related field and/or clinical license (RN, LPN).

Coursework or experience in Quality Improvement, and/or facilitation skills preferred.

Qualified experience in healthcare settings preferred.

Ideal candidate will have had experience in a primary care practice that successfully implemented quality improvement initiatives.

Computer Knowledge :

Excellent computer skills required particularly related to Microsoft applications including Word. Excel, PowerPoint and Outlook. Willingness required to explore and learn new or unfamiliar applications and databases, and when prepared to teach practices and team members how to maximize technology Knowledge of statistical software and other analytic software preferred. Microsoft suite of applications with exemplary Excel/spreadsheet skills.

Pay Details: $57.69 per hour
