Join Our Team as a Community Care Licensed Vocational Nurse (LVN) in Long Beach, CA!
We are seeking a highly skilled and compassionate LVN to join our health team in Long Beach, CA!
Position Details:
Salary: Up to $56,000 annually

Job Responsibilities:
Conduct basic health assessments, assist family members and their families/caregivers in identifying needs, provide basic health information, and make appropriate referrals through home visits and telephonic outreach.
Establish trusting relationships with family members and their families while providing general support and encouragement.
Travel to family members' homes to conduct remote physical exams under the supervision of remote clinicians.
Provide ongoing follow-up, basic motivational interviewing, and goal setting with family members/families/caregivers.
Education:
- Licensed Vocational Nurse (LVN) in the state of California.
