Job Description:

We are seeking an experienced RN Clinical Manager for our Home Health & Hospice team. If you have a positive attitude, enjoy leading and coaching a team, and are comfortable with direct patient care, we encourage you to apply!

Responsibilities:

  • Lead and supervise a team of 8-10 clinicians including RNs, CNAs, chaplains, and social workers.

  • Oversee patient care and case coordination, ensuring seamless scheduling of admissions and quality service delivery.

  • Occasionally cover patient visits to support staff on PTO/FMLA.

  • Coach and precept nurses to ensure quality care and workload management.

  • Facilitate effective communication with MDs and NPs, preparing for interdisciplinary group (IDG) meetings.

  • Organize and manage workflows, including placing DME orders, faxing scripts, and care coordination.

  • Support staff with questions and provide backup assistance to managers.

  • Conduct interviews for new hires and help managers screen potential candidates.

  • Foster a positive, collaborative work environment that prioritizes patient care and team development.

Qualifications:

  • Active RN license.

  • At least 6 months of clinical management or supervisory/leadership experience.

  • Experience in home health or hospice care.

  • Familiarity with the EMR system, Homecare Homebase.

  • Exceptional interpersonal skills, especially in coaching and mentoring clinicians.

  • Strong organizational skills with the ability to handle flexible interruptions.

  • Comfortable working closely with MDs and NPs and communicating side-by-side during IDG preparation.

· If you are positive because of leading clinicians, need leaders that coach and train, heart for hospice, willing to make home visits when unexpected needs arise. We need them to jump in and go into patient care, in field for supervisory fields, precept nurses for extra coaching and workload, not shy when talking with MDs and NPs side by side for IDGs prep team orders.

Day-to-Day:

· Occasionally do visits to cover if someone is on PTO or FMLA, etc. So, they need someone OK with patient care.

· Precept nurses/extra coaching

· Schedule admissions that came in over night

· Patient needs

· Case coordination

· Support staff and answers questions if they need help with something or the manager is out.

· 8-10 members on the team.

· Direct communication with MDs and NPs (very close team). IDGs prep

· Reorganizing à working schedules first thing, processing workflow placing DME orders, faxing scripts, coordination care, supporting staff, qualities flexible interruptions all day long and support staff questions, need help interviews sitting in on that to screen their staff or back managers out doing that hire own aides and case managers work with chaplains social workers supervising everyone CNAs, nurses chaplains, social worker 8-10.

Pay Details: $90,000.00 to $93,000.00 per year
